It is our Mission to provide expert care, comfort and support for those facing life-limiting illness through direct support of patients and their families, and public education and advocacy.

What is Hospice?

Hospice is internationally recognized as a program that delivers personalized care and services in the home to persons determined by a physician to have less than six months to live. Hospice workers are trained to become closely involved in listening to and carrying out the patient's wishes, in providing comfort and in symptom management.

To Our Patients' Physicians and Partners in Care: Ease of refferal and timely appropriate communications is the hallmark of our service.  We value our collaboration in meeting patient and family needs and strive to warrant your continuing confidence. 

 

What Services are included?

 

Nurses on call 24 hours, 7 days a week

Medications, treatments and equipment are delivered to the home

Assistance for caregiving including bathing, dressing and grooming; and homemaking as needed. 

Volunteers to sit with or read to patients

Help in accessing community resources and in financial planning

Spiritual and psychosocial counseling

End of life planning

Family bereavement support for a year

 

What is the Family's role?

 

The family and/or caregiviver continue to provide care as before with the assistance of the Hospice Team.  The family helps the Team undersand and carry out the patients wishes.

 

Where Can the Patient Reside?

 

"HOME" is considered to be wherever the patient lives.  It can be a personal residence, a board and caring assisted living home or a nursing home.  If needed, the Hospice team can help with facility placement or respite stays.

 

How much will Hospice cost?

 

Hospice services are covered by Medicare, Medicaid and many insurance companies.  Medications, treatments and services unrelated to the terminal diagnosis continue to be reimbursed as usual.

 

How are Hospice Services Started?

 

The patient, family or caregiver can call for a meeting with Hospice

Referral calls can also be made by, Physicians, Hospital or nursing home social workers and Clergy.

The patient and family can then decide to elect Hospice Services.

 

Our hospice services are available regardless of age, race, color, sex, religion,

sexual orientation, ethnic or national origin, disability, diagnosis, ability to pay or availability of a caregiver.
